Honestly, you would be hard pressed to find all that many fatties in the positions you described. Especially doctors.

Instead of flat out killing them, the best case scenario is to have something similar to indentured servitude. Put all the fatty vermin to work out in the sun until they reach a human body type.

I'm not sure I have ever seen a fat doctor or nurse in my life and my dad was a doctor and I've met many of his coworkers.
