he wasn't a prick, but i could see people mistaking him for one. sort of like how people who can't tell facetious, waggish, sardonic (etc) apart will often call any of those things "sarcastic."

letterman wasn't a mean dude, but he didn't suffer fools lightly. i think he could tweak most people without them really knowing, except for the pampered, famous ones, who could probably tell just because they weren't getting the red carpet treatment they were used to. madonna was a famous example of that, but i'm sure he made other enemies.
