I think the scam was:

1. NBA players get fake paperwork for surgeries that they weren't actually going to have done
2. Doctors are in on it, sign the paperwork and submit it to the NBA insurance
3. NBA insurance pays the doctors for the fake medical work
4. Doctors split the cut with the players

This is why insurance companies pay investigators. I wouldn't be surprised if there was an organized crime/blackmail element to this as well.
