We have a guard in our rec league who uses it extensively along with pump fakes. He's only average at it percentage wise but seems to be able to release it every single time, even when the opponent is way taller and is expecting it. Almost never get blocked. You don't need to be Kareem's height to use it effectively.

Probably because that isn't the traditional way of shooting a basketball so nobody practices it. As a child your shooting with both hands be it granny style or not.

It’s a difficult shot to hit efficiently. Time is better spent working on other moves in most cases.
